It also allows residents to adapt to any lifestyle changes they’ve experienced. By the end of this care, your loved one should be able to continue leading a normal lifestyle. Before entering a care home for rehabilitation, you or your loved one will have a comprehensive care plan created outlining care needs and the goals you’ll achieve to regain independence.

Can you get six weeks of free rehabilitation care?

In the UK, you can get up to six weeks of free rehabilitation care after leaving the hospital (this is often referred to as intermediate care or reablement). This short-term care aims to help you regain your independence. The NHS and/or your local authority arrange and funds this care.

Your condition will be assessed before being discharged from the hospital to determine if you’d benefit from temporary rehabilitative care. While this can last for up to six weeks, it often only lasts for just one or two weeks, depending on how quickly you recover.
